Revoking the Hospice Benefit

In some cases, a client may opt to revoke their hospice benefit and pursue a more aggressive or curative form of care. This is completely within the rights of each hospice beneficiary. To revoke the hospice benefit, you will need to sign a simple form on the day of revocation. If you decide to go to the hospital or emergency room, please contact a member of your hospice team or the Dignity Hospice of Utah office immediately. A hospice representative can then meet with you and you can sign the revocation form. This will prevent Medicare from denying payment for your hospital visit and leaving you with a large medical bill. In emergency cases when 911 is contacted, please alert a hospice representative after you have reached the hospital.
